If you wish to be eligible as an Athlete or Unified Partner to participate for the 2023 Special Olympics Georgia Summer Games, then It’s time to start completing the paperwork which has a Friday, March 24th deadline. The games will be held at Emory University on May 19-21, 2023.
Here is a list of the required paperwork for this year’s Summer Games: All needed forms are available on our web site home page under Resources then click on Forms.
Athlete Medical Packet:
- Participant Release Form– Sign and date.
- Medical Form-Athletes will need to have an updated medical. (Be sure the doctor signs and dates the Physicians Form.) The medical forms are good for 3 years. All signed and completed medical forms will need to be turned into Special Pops Tennis and a copy sent to Special Olympics GA by March 24, 2023.

Volunteer & Unified Partner Profile Form: The Profile form and the Code of Conduct form only need to be signed once. All new Unified Partners will need to fill out this form. Volunteers over the age of 18, please complete the Background screening and Protective Behaviors. These are good for 3 years.

Waiver and Release of Liability, Assumption of Risk and Indemnification Agreement for Communicable Diseases: Please check all the boxes and sign. This form is also a 3 year form. If you signed this form and have this on file with Special Olympics, you will not need to sign this again to participate at this year’s games.
REMEMBER: March 24th is the Deadline for the Athlete medical forms and the Volunteer & Unified Partner Profile Form to be sent to:
